Vital HealthCare is a Domiciliary Care Agency with community rehabilitation and supported living resources Nationwide. We specialise in providing holistic and comprehensive care packages for people with learning disabilities, aspergers/autistic spectrum disorders, acute Brain injury and mental health needs. Our service mainly focuses on providing tenancy based community rehabilitation and intensive support for adults who have required low-medium secure care for long periods. Intensive support is provided through high standard domiciliary care and placements that have been carefully chosen close to public transport, shops and other services to ensure greater integration into the community.

The Team

Our team consists of Nurses, Psychotherapists and NVQ level staff.

Bespoke Care

Vital Healthcare provides individualised packages of care which include but are not limited to - Community tenancy based rehabilitation, supported living, live-in care/support, ECI (embedded clinical input).

Specialist Care

Vital Healthcare also provides bespoke care packages for Brain injury, Downs syndrome, dementia, palliative care and other specialist care.

Assessed Needs

Based on individually assessed needs, the staff will provide a range of services which include supervision, support with personal care, activities of daily living, nursing care, support with planning daily activites and much more.

We endeavour to help people to access mainstream services, acquire skills to enrich their lives and seek a good quality of life, a healthy lifestyle, access to leisure activities and high standards of care and support.

'Community,

The CRTL-Community Rehabilitation & Transitional Living, is a model that brings about all the social, clinical and organisational strands together.

Rehabilitation,

It uses the strengths of "supported living", the principles of "person centred planning", especially social inclusion and a better quality of life, with health promotion and involvement in mainstream community life.

Transitional,

The aim of this model is to create purposeful, inclusive and active lifestyles in the community, to develop care packages and to cater for an individual's needs.

Living.'

But, with such robust safeguards that prevent readmissions due to deterioration in mental and emotional health, into any residential units; and these packages are, on average 25% less expensive, from day one, as compared to the residential facilities like intensive care, high dependency, low and medium secure units and hospitals.
